Key Responsibilities:
Support the Quality Assurance and Assessment Committee by helping implement corrective action plans for quality-related issues, as directed.
Participate in discharge planning and contribute to the development and execution of activity care plans and resident assessments.
Arrange transportation for residents as needed for outings, medical appointments, or discharge.
Assist in developing and maintaining a monthly calendar of activities that includes:
In-room activities tailored for bedbound or socially isolated residents
Encourage residents to pursue self-initiated hobbies and provide necessary materials (e.g., crafts, reading materials, Braille or audiobooks).
Assist with documentation, including assessment records and quarterly progress notes as assigned.
Maintain a clean, organized, and secure activity department area.
Qualifications:
High school diploma or equivalent required.
One year of experience in a long-term care or similar healthcare facility preferred.
Skills & Abilities:
Ability to understand and follow technical instructions and policy/procedure manuals.
Strong communication skills to interact effectively with staff and residents.
Basic mathematical competency, including knowledge of fractions, percentages, and ratios.
Ability to interpret and apply written, verbal, or diagrammatic instructions in various formats.
Problem-solving skills and adaptability in a dynamic care environment.
Physical Requirements:
Frequent standing, walking, reaching, talking, and hearing.
Occasional sitting.
Frequent pushing/pulling and use of hands and arms.
